The death of an Indigenous mental health patient at Royal Darwin hospital last year was avoidable and due to “a litany of failures”, a coronial inquest has been told
Ngalarina, a sistergirl also known as Xysc after changing their name in 2017, was an involuntary mental health patient at the hospital in 2022 when they died alone in a bathroom from volatile substance abuse.
The Northern Territory coroner asked media to use Ngalarina and they/them pronouns in line with their family’s wishes
